Most Insurance plans will cover the FDA cleared ATLAS-T® Pillow. We work directly with your insurance provider for reimbursement and there is zero risk to you. This is not a Gimmick. Follow these simple steps to find out if your insurance benefits will pay for your ATLAS-T® Pillow:
1. Complete the Online form below. Once completed, you will be redirected to our Home Page. You may also fax a copy (front and back) of your insurance card and Date of Birth to our secure fax: 1-253-799-3853. Please include your email address and a telephone number that we can reach you. Or you may email us your Insurance Provider's Name, ID Number, Phone number of insurance provider and your Date of Birth to customerservice@atlastpillow.com.
2. We generally contact you with in 24 hours and let you know if you qualified.
3. When approved, we will email you the necessary forms.
4. The ATLAS-T® Pillow will be dropped shipped to you usually with in one week.
